Returning Students

1. Application for Bureau, Federal and Grant Funded School – Form 6248
2. Immunization Record w/ CURRENT School Year (prefer Indian Health Service Copy)
3. Provide Legal Guardianship documents (if necessary)
4. Power of Attorney for current school year
5. Court Document for Legal Guardianship
6. Student Health Packet
7. Physical Examination per student is required, please schedule PE for your child(ren)
8. Home Language Survey Form
9. Title VII Student Eligibility Certification

New Students

1. Application for Bureau, Federal and Grant Funded School – Form 6248 (this application is a new version and it’s a requirement from Arizona Central Office)
2. Birth certificate
3. Social security card
4. Certificate of Indian Blood
5. Immunization Record w/ CURRENT School Year (prefer Indian Health Service Copy)
6. Provide Legal Guardianship documents (if necessary)
7. Power of Attorney for current school year
8. Court Document for Legal Guardianship
9. Student Health Packet
10. Physical Examination per student is required, please schedule PE for your child(ren)
11. National Lunch Application ( not available until July 2018)
12. Transferring Students MUST provide transcripts from last school attended
13. Home Language Survey Form
14. Title VII Student Eligibility Certification
